Dear Council of School Officers Retiree:
It is now time to renew your membership in the CSO Retirees Chapter, along with your Vision Coverage for 2022.
The annual renewal cost for Membership is ONLY $60.00.
The annual renewal cost for Vision benefits and your membership renewal is $256.68.
UnitedHealthcare Vision is pleased to offer you the chance to renew your Vision benefit membership in the CSO program effective January 1, 2022, through December 31, 2022.
Vision membership must be renewed each year. Your renewal period is from November 8, 2021, to December 13, 2021. Renew now to enjoy the benefits for the entire year. Retirees are on a calendar year benefit. Your current benefit will expire December 31, 2021. Vision membership renewal must be received by December 9, 2021.
Your Vision renewal covers the cost of well vision care and corrective lenses and eyewear with affordable premiums, as well as lower out-of-pocket expenses. The network includes more than 63,000 private practice and retail providers of vision services. For 2021, the vision plan will cover a $150 contact lens benefit in lieu of glasses. If you choose glasses, all levels of progressive lenses are covered in full at an In-Network provider (Standard, Deluxe, Premium and Platinum) as well as, tints/uv coatings and standard anti-reflective coating.
Included with your membership dues, CSO is now offering a Partially Funded Legal Services Plan. This Plan provides legal advice and representation to retirees and eligible members of their families in the basic areas of the law:
Domestic Relations, Wills, Contracts, Consumer Problems, Personal Injury, Criminal, Juvenile Proceedings,
Traffic Cases, Credit Problems, Real Estate, and Civil Litigation.
A fully prepaid legal plan is available for an annual fee of $300.00 to be paid by the individual member. You can find more information on the legal plans here.
